SSVF Supervisor (Full-time)

For over 75 years, Catholic Family and Community Services, has provided at little or no cost, a full spectrum of services to individuals who are oppressed, families who are in crisis and communities who are disenfranchised.

Located throughout the New Jersey Counties of Passaic, Morris and Sussex, we provide professionally based social services, including Behavioral Health Programs, Veterans Services, Children’s Programs, Senior and Aging Services and much more.

SSVF Supervisor

Schedule per week: Monday - Friday 8:30am - 4:30pm (flexibility needed)

Location: Paterson, NJ

SSVF Program Supervisor is responsible for the organization of activities and functions to ensure the case managers are in line with the goals and objectives specified for the program and are accomplished in accordance with established priorities, time limitations, funding limitations or other specifications. The primary responsibility of the program manager is to oversee and support the assigned case manager’s day to day activities as it relates to the SSVF Program. The Program supervisor is responsible to attend and coordinate with community providers. SSVF Supervisor can also maintain his/her own caseload as needed

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  1. Have a working knowledge of all aspects of the SSVF Program Guidelines and CFCS SSVF Policies and Procedures
  2. Supervise and direct case managers to ensures that veterans receive a high quality of care
  3. Maintain their own caseload as needed
  4. Manage the day-to-day operations of the case managers in our catchment areas (Passaic, Morris, Sussex, Union, Hudson, Bergen, and Essex Counties)
  5. Review and approve all SSVF intake files for eligibility
  6. Ensure that clients receive appropriate services as required by our SSVF contract and SSVF Program Guidelines
  7. Follow Housing first approach to housing
  8. Review and approve all financial assistance requests submitted by case managers before submitting to the SSVF Director for final approval. In addition, ensure all requests are accurate and timely.
  9. Participate in all VA calls and trainings as assigned.
  10. Provide monthly statistical information
  11. Conduct bi-weekly supervisions with case managers
  12. Ensure eligibility screenings, intakes, contacts, housing stability plans, temporary financial assistance, and other information is documented timely and accurately in the electronic health record (AWARDS)
  13. Function as a team member by covering co-worker's responsibilities, participating in program activities, and communicating relevant information to other team members.
  14. Develop and maintains relationships with local social services, public benefit agencies, shelters, faith based and community-based organizations.
  15. Collaborate with the Continuum of Care (COC) in our catchment areas and other VA, and community providers to ensure services are delivered efficiently.
  16. Actively participate in the Veteran By- Name List efforts in collaboration with the COCs and other VA and community providers in our catchment areas.
  17. Attend and participate in all staff meetings, trainings, and case conferences, as assigned
  18. Ensure staff are documenting service delivery accurately and timely while ensuring veteran confidentiality
  19. Communicate with staff and clients to resolve issues that may arise
  20. Conduct chart reviews
  21. Ensure all client files contain all required documentation as per the SSVF Guidelines
  22. Have a working knowledge of AWARDS/HMIS database
  23. Monitor all screens to ensure contact is made within 24 hours of receipt of screen
  24. Ensure all data is entered accurately and timely into the AWARDS database
  25. Transport clients in agency vehicles in a safe, cautious, and responsible manner
  26. Communicate all barriers and/or issues to SSVF Director
  27. Submit monthly reports as required
  28. Adhere to agency policy, procedures, and professional code of ethics
  29. Responsible for other duties that the SSVF Director may assign from time to time to support the effective and efficient operation of Catholic Charities/SSVF.

KNOWLEDGE, SKILLS AND ABILITIES:

Work requires experience and knowledge in the following:

  1. Bachelor’s Degree required
  2. Commitment to serving homeless individuals
  3. Minimum of 2 years’ experience in providing case management
  4. Experience in program management and staff supervision
  5. General knowledge of local community resources
  6. Proficient in Microsoft Office
  7. Good verbal and communication skills
  8. Ability to work independently
  9. Ability to multitask, maintain confidentiality, and meet deadlines
  10. Ability to maintain professional conduct, attitude, and appearance at all time
  11. Ability to identify underlying challenges and barriers
  12. Ability to have attention to detail
  13. This position requires a Valid Driver’s license with a good driving record
  14. Must be a proactive critical thinker and problem solver
