Case Manager - Hope House

Located throughout the New Jersey Counties of Passaic, Morris and Sussex, we provide professionally based social services, including Behavioral Health Programs, Veterans Services, Children’s Programs, Senior and Aging Services and much more.

Case Manager - Hope House

Hours: Full Time Monday - Friday 8:30am - 4:30pm

Location: Dover, NJ

Regular Salary: $23.00/Hr

Job Summary:

Individual provides non-clinical support services to consumers within the specific population living in the community who are in need of supportive services such as home assessments, individualized care plans, coordination of care team, resource information and referral, linkage to insurance and/or long term care services.

Position Specific Job Duties and Responsibilities (Essential Duties):

1. Assess clients with community supportive living needs related to optimal health, safety and well-being.

2. Educate and empower clients to manage their chronic disease needs.

3. Identify interventions and implement a plan of care to address client needs and ensure safe community living.

4. Perform screenings every six months, as indicated by client risk factors.

5. Coordinate program contract requirements and delivery of units.

6. Prepare all monthly, quarterly, year-end reports.

7. Assist with grant application preparations as described.

8. Participate in strategic planning and quality improvement activities including data collection, ongoing monitoring and improvement initiatives.

9. Responsible for other duties that may be assigned from time to time to support the effective and efficient operation of Catholic Family and Community Services.

Qualifications:

1. Bachelor’s Degree or associates degree with 3-5 years’ work experience preferred. Prior experience in human services preferred.

2. Bilingual; Spanish speaking preferred.

3. Will require training in HIV Medical Case Management (MCM) Training Program

4. Ability to work independently.

5. Ability to serve as a liaison between consumer and third parties.

6. Basic computer skills including accuracy of spelling and computer data entry.

7. Excellent interpersonal and communication skills (verbal/written) in English, strong organizational abilities, and a high degree of professionalism. Bilingual English/Spanish is preferred. Good telephone communication skills

8. Experience or willingness to build community partnerships and the ability to communicate/connect with a diverse population. Provide a high level of customer service

9. Ability to utilize necessary technologies to provide and document services virtually.

10. Establish and maintain effective working relationships with clients, families, caregivers, service providers and staff

11. Must possess a valid NJ driver’s license, access to a reliable vehicle (mileage reimbursement provided) and willingness/ability to travel to designated sites
